Sign in at app.getconduit.co and use the secure message thread in your case. It reaches your care team, it is the fastest route, and it is the only channel where your health information belongs. Cancelling is in the portal too — immediate, no retention call.
Privacy and data rights
Use the form below and choose “Privacy request,” or write to the postal address. We respond within 45 days, as our Privacy Policy and Consumer Health Data Privacy Policy commit. Members can also send rights requests through the portal, which verifies identity faster.
